Adoption Contract and Receipt
• Adopter agrees that dog will be kept as a home companion and will share the adopter’s home.
• Adopter will not use, nor will he/she allow the dog to be used for any illegal purpose.
• Adopter agrees that the dog will not be attack or protection trained and will not be used for guard work in any commercial endeavor.
• Adopter agrees the dog will not be used for medical or experimental purposes.
• Adopter agrees that if for any reason the adopter is unable to keep this dog for its entire life that said adopter must contact 4 Paws Society and make arrangements to return dog to 4 Paws Society.
Adopter agrees not to sell, give away or dispose of the dog (except euthanasia for medical reasons at the advice of a licensed veterinarian) without prior consent in writing from 4 Paws Society.
• Adopter accepts this dog as is with all defects, either observable or unobservable and assumes all risk for such dog from the date of signing this contract.
• Adopter agrees to care for the dog in a humane manner, including but not limited to providing proper food, water and adequate shelter.
• Adopter agrees to provide veterinary care including annual inoculations for the duration of the dog’s life.
• Adopter agrees to provide monthly heartworm prevention on a year round basis.
• Adopter agrees that under no circumstances is the dog to be chained or tethered as a means of confinement.
• Adopter agrees that at no time will the dog be transported in the open bed of a pick-up truck or left in a vehicle when outside temperature exceeds 70 degrees Fahrenheit.
• Adopter assumes all responsibility for the dog’s actions after the effective date of the adoption from 4 Paws Society. Adopter agrees to hold 4 Paws Society and its representatives harmless from any and all liability associated with any illness of the dog or damage or injury caused here after by said dog.
• Adopters pledge to give the new dog a minimum of two weeks to adjust to the new home.
• Should the adopter breech the Adoption Contract, 4 Paws Society shall have the right to rescind this adoption contract and the right to take possession of the dog. If the Adoption Contract is rescinded or revoked then a representative of 4 Paws Society or a member of city or parish law enforcement shall be entitled to take immediate possession of the dog and remove the animal from any premises in which it is housed and entry shall not constitute trespass.
Adopter agrees to forfeit all amounts paid to 4 Paws should a breech of the Adoption Contract occur. If attorney fees, court costs are incurred to recover the dog, the costs shall be the burden of the adopter.
